Stretches to Treat Your Nerve Pain

Hi this is Dr. Silvester and I’m going to show patients how to do a thing that we call nerve gliding here at the clinic. It’s an exercise we use for all kinds of neuropathology.  A lot of people have Neuropathy and we use it for patients with Neuropathy, and we also use it after surgery if you have Tarsal Tunnel Syndrome. If you have Tarsal Tunnel Syndrome, we use it before surgery to try and get it to go away. \n\n   \n\n Basically, it’s stretching the nerve and getting it to glide. So I’m going to have my assistant here move her foot and we’re going to show you where this Tarsal Tunnel is. \n\n   \n\n The nerve comes down here like this and then it branches out into three big branches and then a couple depending on the individual variability of these other branches as well. And then this branch is quite small. But anyway, there’s a tunnel that this nerve gets trapped in... there’s a little thing up here called the Flexor Retinaculum, this is called the Tarsal Tunnel, and then there are some other tunnels down here over the nerve. The nerve kind of gets pinched there. So, one of the things that we do is try to get this   nerve to move in this tunnel. So, I’ve drawn it so you can see it move all the way in and the nerve is fairly short. And then you move it all the way out and the nerve gets very long. You can see a “gliding” going on alongside that foot. And so, what happens is that we’re trying to get the nerve to loosen up and glide, and thereby decrease the adhesions and things. \n\n   \n\n How we tell our patients to do it is to take the foot up and out this way (hold it for 30 seconds), and then bring it down and in and hold for 30 seconds. And then repeat that about 5 or 6 times, and then do that maybe 10 times a day: up and out, down and in, do that 5 times, and then do that 10 times a day. Okay? \n\n   \n\n There’s another nerve on this leg and it comes down here on the leg and goes down. You can do that nerve as well, which is typically the same exercise but you’re just stretching the nerves on the front of the leg. And so it’s down and in, you stretch it for 30 seconds, and then up and out. The symptoms that can go along with this nerve pain on the top of the leg you have are numbness, burning, electrical sensation, on the top of your foot in this area. On the bottom of your foot when you have that, it can come as pain, it can come as arch pain, it can come as just nighttime burning, tingling - all the symptoms that people associate with Neuropathy. \n\n   \n\n So anyway, this is a nerve exercise you can do, and if you do it frequently a lot of times it can alleviate the pain that you have in your leg. We use it all the time right after surgery to keep the nerves moving so the scar doesn’t heal down on the nerve. Thanks for listening.  \n

Peripheral neuropathy: Symptoms, causes, and treatment Overview Peripheral neuropathy, a result of damage to your peripheral nerves, often causes weakness, numbness and pain, usually in your hands and feet. It can also affect other areas of your body. Your peripheral nervous system sends information from your brain and spinal cord (central nervous system) to the rest of your body. Peripheral neuropathy can result from traumatic injuries, infections, metabolic problems, inherited causes and exposure to toxins. One of the most common causes is diabetes mellitus. People with peripheral neuropathy generally describe the pain as stabbing, burning or tingling. In many cases, symptoms improve, especially if caused by a treatable condition. Medications can reduce the pain of peripheral neuropathy. Peripheral neuropathy care at Mayo Clinic Symptoms Every nerve in your peripheral system has a specific function, so symptoms depend on the type of nerves affected. Nerves are classified into: Sensory nerves that receive sensation, such as temperature, pain, vibration or touch, from the skin Motor nerves that control muscle movement Autonomic nerves that control functions such as blood pressure, heart rate, digestion and bladder Signs and symptoms of peripheral neuropathy might include: Gradual onset of numbness, prickling or tingling in your feet or hands, which can spread upward into your legs and arms Sharp, jabbing, throbbing, freezing or burning pain Extreme sensitivity to touch Lack of coordination and falling Muscle weakness or paralysis if motor nerves are affected If autonomic nerves are affected, signs and symptoms might include: Heat intolerance and altered sweating Bowel, bladder or digestive problems Changes in blood pressure, causing dizziness or lightheadedness Peripheral neuropathy can affect one nerve (mononeuropathy), two or more nerves in different areas (multiple mononeuropathy) or many nerves (polyneuropathy). Carpal tunnel syndrome is an example of mononeuropathy. Most people with peripheral neuropathy have polyneuropathy. When to see a doctor Seek medical care right away if you notice unusual tingling, weakness or pain in your hands or feet. Early diagnosis and treatment offer the best chance for controlling your symptoms and preventing further damage to your peripheral nerves. Causes Not a single disease, peripheral neuropathy is nerve damage caused by a number of conditions. Causes of neuropathies include: Alcoholism. Poor dietary choices made by people with alcoholism can lead to vitamin deficiencies. Autoimmune diseases. These include Sjogren's syndrome, lupus, rheumatoid arthritis, Guillain-Barre syndrome, chronic inflammatory demyelinating polyneuropathy and necrotizing vasculitis. Diabetes. More than half the people with diabetes develop some type of neuropathy. Exposure to poisons. Toxic substances include heavy metals or chemicals. Medications. Certain medications, especially those used to treat cancer (chemotherapy), can cause peripheral neuropathy. Infections. Traumas, such as from motor vehicle accidents, falls or sports injuries, can sever or damage peripheral nerves. Nerve pressure can result from having a cast or using crutches or repeating a motion such as typing many times. Tumors. Growths, cancerous (malignant) and noncancerous (benign), can develop on the nerves or press nerves. Also, polyneuropathy can arise as a result of some cancers related to the body's immune response. These are a form of paraneoplastic syndrome. Vitamin deficiencies. B vitamins — including B-1, B-6 and B-12 — vitamin E and niacin are crucial to nerve health. Bone marrow disorders. These include abnormal protein in the blood (monoclonal gammopathies), a form of bone cancer (osteosclerotic myeloma), lymphoma and amyloidosis. Other diseases. These include kidney disease, liver disease, connective tissue disorders and an underactive thyroid (hypothyroidism). Peripheral neuropathy is basically when you have damage to your peripheral nerves from either a trauma or disease. This can cause your hands and/or feet become numb and tingly. I'll focus on the feet/legs for this video. See Doctor Jo’s blog post about this at: http://www.askdoctorjo.com/peripheral-neuropathy-feet-legs Peripheral neuropathy can be caused from diabetes, medications, or even chemotherapy, as well as many other things. Having good flexibility and strength in these areas will help. The first stretch will be a calf stretch. Sit on the ground with your legs straight out in front of you. You can bend the leg you are not stretching towards you. Put a belt, towel, or dog leash around the ball of your foot. Keep your back and knee straight, and relax your ankle. Pull your foot towards you with the strap until you feel a stretch. Hold this for 30 seconds, and do it three times on each side. Next are ankle pumps. With your legs straight out in front of you, place a roll just under your ankle so your heel can move freely. Trying to keep your leg straight and moving only at your ankle, pull your foot up towards you, and then push it down like you are pushing on a peddle. Then you will do ankle circles. Again, try not to move your whole leg. Keep the movement at the ankle. Do ten one direction, and then reverse it and do ten the other direction. Now you will do a simple straight leg raise (SLR). You want to squeeze your muscles tight to lock out the knee and pull your toes towards your head to keep the whole leg straight. This will work your hip flexor muscles when you lift your leg off the ground. Use slow controlled movements to make sure you are using the muscles and not momentum. Start off with ten, and work your way up. Then, you are going to lie on your side. The top leg is going to stay straight and pull your toes up towards you. Keep your body in a straight line as well. This is going to work your hip abductor muscles. Then you are going to work the bottom leg working your hip adductor muscles. Same as above, keep the leg straight. Start with ten of each of these, and do both sides. In standing, you will do a heel/toe raise off the ground. Stand with your feet about shoulder width apart, come up on your toes as high as you can. Try not to lean forward, but bring your body straight up and slowly come back down. Push off as much as you can so your heel leaves the ground. Stand on one foot, but hold onto something sturdy. Try to balance for 30 seconds to a minute. When that becomes easy, just use one finger one each side. Then just one finger for balance, and finally try balancing without holding on at all.
